This patient has a "left-sided" supply type, i.e. the lateral wall branch (circumflex branch) supplies not only the lateral wall but also the posterior wall. The last of these branches shows severe constriction close to the exit. Electrocardiographically, an infarct caused by this stenosis would be expected to cause mainly changes in the posterior wall region, perhaps also in V6.
