Overt heart failure can be diagnosed by clinical means while the clarification of the etiology and the appropriate choice of treatment modalities necessitates further diagnostic procedures. Diagnosis has
to clarify whether there are any pathophysiological processes amenable to treatment of the underlying pathology or whether treatment has to
aim at the relieve of symptoms and the improvement of prognosis by inhibition of further re-modelling.
